Sight Service, Inc. The financial guidelines
to qualify for our service can be viewed or downloaded
here.
The interviewing Lion should meet with the patient and
complete
the form totally,
then contact the Middle Tennessee Lions Sight Service Office.
It is important that you fill out the form completely.
Mail the completed form to the Middle Tennessee Lions Sight Service. We will make the appointment and
notify the Interviewing Lion and the patient by mail regarding the date and time
of appointment. The patient’s first
visit to the
Tennessee Lions Eye Center
at Vanderbilt is for evaluation purposes ONLY; please do not expect surgery
unless it is a dire emergency. The
medical staff at Vanderbilt makes the medical decision when and if surgery
is needed. Please remember, MTLSS funds should only be used as a
last resource and then only
to prevent complete loss of sight.
Middle Tennessee Lions Sight Service depends on Lions
Clubs contributions to White Cane Funds to continue providing eye care to the people
of District 12-I and 12-S. Donations
and memorials also allow us to do a little extra to help.
All donations received marked for Low
Vision will go into that special line
item account and will be used for low vision only.
